Paraclete Home Health Care is an agency owned by two advanced practice nurses. It is licensed by the New York State Department of Health. We are insured and bonded. Our multi-disciplinary team works closely with families and their primary care physicians to create and implement a personalized written care plan, tailored to their specific needs as well as providing the care they deserve in the comfort of their own homes.
