Ultimate Grace works with seniors and their families to create a customized care and wellness program that benefits everyone, and our trained care managers implement that plan as a loving and grace-filled companion.
Our care managers perform a variety of daily activities, and coordinate with other agencies, providers, and wellness resources to meet all a senior’s health care needs. We transport seniors to doctors appointments, take them shopping and accompany them to family gatherings and other outings.
Ultimate Grace LLC is responsible for hiring and monitoring aides and is fully insured. Our priority is to provide exceptional care for our seniors & total peace of mind for their families by creating a relaxed environment where everyone can enjoy time together, without worry.
We also provide hospice care in the home or at one of our boutique alternative hospice spaces. Michael’s House in Brighton, NY and Ultimate Grace House in Penfield are private, family-centered facilities that focus solely on meeting the many emotional and spiritual needs during this difficult life transition.
