Concierge Case Management
Since 1990, The Caregiver Resource Center’s mission has been to work with clients, their families and healthcare professionals to help assess, plan for and implement ways to allow for their greatest degree of health, safety, independence, and quality of life.
We are unique in that we are available for our clients whenever they need us.
We provide our clients with customized case management and advocacy services 7 days a week by appointment, and 24/7 for emergencies.
Our services are provided on-site in the home, doctor’s office. ER, Hospital or residential care facility.
Typical Calls We Receive
• Daughter needs advocacy services for her dad who’s in the hospital
• Son questions whether mom is safe to live at home alone
• Man seeks help to coordinate his partner’s multiple care providers
• Family struggles to find services for a teen with schizophrenia
• Senior who wants to remain at home, but is struggling
• Couple who want help to move from FL into a CT assisted living facility
• Husband who seeks help for his wife who had a recent stroke
Seniors and people with special needs often face multiple challenges, and require a team approach.
If you think of this team as an orchestra, the team consists of multiple members; doctors, physical therapist, occupational therapist, counselor, or agencies providing services such as caregivers, transportation, etc.)
As with the orchestra, a key member of the care team is the conductor, who is in most cases is a certified case manager.
The client and their family select the services that they want, and they only pay for those services.
