**The PACE approach**
Your care is provided by an Interdisciplinary Team (IDT) that is assigned to each participant consisting of the following staff:
– Physician
– Registered Nurse
– Home Care Coordinator
– Masters level Social Worker
– Occupational Therapist
– Physical Therapist
– Recreational Therapist
– Registered Dietitian
– PACE Center Manager
– Certified Nurse Assistant
– Transportation Driver
The IDT assesses each participant’s needs and delivers services based on a customized care plan. Where needed, the plan will include acute care, hospitalization and nursing facility.
