The Southern Delaware COPD Coalition is a 501c3 non-profit organization committed to improving the quality of life for Delawareans living with COPD and other chronic lung diseases by eliminating health disparities and improving access through health equity, education, advocacy, and bridging gaps in transitional care.
The Southern Delaware COPD Coalition relies on collaborative and innovative relationships across the healthcare continuum to facilitate health equity, continuity and empowerment – and subsequent improvement in quality of life – for Delawareans living with chronic lung disease.
Our Registered Respiratory Therapists perform in-home visits and perform a comprehensive assessment to identify and address barriers to care. Once identified, our team works collaboratively with a multitude of community partners to address patient-specific needs. We routinely work with local hospitals and healthcare systems (Primary Care, Pulmonology and Cardiology offices) as well as social services agencies in efforts to address identified needs impacting our patient’s overall health and well-being.

We also:
o Provide assistance with navigating the social service system (health insurance, prescription insurance/assistance, utility assistance, food assistance, etc.)
o Scheduling appointments and testing
o Addressing any additional identified needs
We treat each patient as an individual to ensure patient-specific needs are identified and addressed.
