Providence Home Health Care, Inc. recognizes that all persons possess a unique hierarchy of needs. Each person searches for meaning in personal life experiences and the choices and decision made are based on that person’s individual values and benefits. A primary goal of Providence Home Health Care, Inc. is to provide intermittent (or part-time) Skilled Nursing, Physical Therapy, and Home Health Aide services to the patient in the patient’s place of residence.
Patient care is an ongoing process of evaluating, treatment planning, goal setting and are re-evaluation. The care plan will be developed in cooperation with and participation from the patient and/or family to meet each patient’s unique needs.
