Paired Health, LLC and Paired Health Provider Group, PA started in 2012, with a focus on helping patients successfully transition from the hospital to the home and ultimately to their primary care provider. The Paired Health name and Better. Together. mission statement embodies the philosophy that partnering together results in better health outcomes for patients, hospitals and the community.
Paired Health Provider Group, PA is a company founded by physicians, so our emphasis is on encouraging proactive, physician directed care immediately following a hospital discharge or ED visit and offering outstanding programs and services that can truly make a difference. We believe in giving our patients the best care, rapidly and consistently, regardless of the barriers they face.
Our management and clinical team has a long history of working with hospitals and community health providers in North Carolina to successfully care for many of the most medically challenging patients and transition them from inpatient care to a primary care home.
