Program of All-Inclusive Care for the Elderly providing innovative healthcare to frail seniors wishing to age in place in their home environment. Research shows that care coordination for this frail population prevents failure to thrive syndrome and breaks the cycle of recurrent hospital ER visits. By providing medical care, physical/occupational therapy and constructive socialization and by utilizing a full interdisciplinary team of professionals, seniors and their family/caregivers are fully supported to remain at home until end of life.
