We are a multidisciplinary team that provides customized training designs, professional development and e-learning solutions centered on culturally competent care, inclusion and health equity strategies. Our Inclusion Scorecard for Population Health is an online dashboard of over 70 best practices to develop a coordinated strategy to reduce health inequities. This customized solution can be used to engage leaders in population health, patient services, quality, community benefits, human resources, and equity to target key activities across a health system. We are also a pioneering organization in designing and managing Pay for Success initiatives focused on chronic conditions.
