Elites Care is a private family owned for-profit corporation. We are a one stop health care company, dedicated to providing a variety of health care services, ranging from helping persons with intellectual and other developmental disabilities through its Person and community support services and programs, to providing basic chore and homemaker services. Elites Care’s mission is to provide personalized community support and Person support services to persons with developmental disabilities, intellectual disabilities and substance abuse. These supports and services are designed to maximize and maintain living skills and assist each person to achieve a greater degree of social and economic independence. Services will be provided in accordance with the goals outlined in the beneficiaries Support Plan. Each beneficiaries support plan is developed jointly with the person and the people important in their life. Elites Care’s goal is to provide effective services and supports with results that satisfy the person with the input of his or her family, friends, agency employees and any other person(s) who are important to the life of the beneficiary. Elites Care will provide these services to eligible beneficiaries without regard to race, color, creed, national origin, gender, marital status or sexual orientation.
