CarePoint Home Health, LLC is a 245D Home & Community-Based Waiver Services provider offering both Basic and Intensive care. Our Personal Care Assistance Agency services include the PCA Traditional Option as well as the PCA Choice Option. CarePoint is fully licensed and enrolled through the Minnesota Department of Human Services.
