Care Directives-through its Advance Directive information Exchange (ADiE) platform-has collected, organized and served Advance Care Planning (ACP) documents across the care continuum to thousands of providers since its founding in 2016. The growing network of hospitals, health systems, skilled nursing facilities, EMS, and providers utilizing ADiE to communicate patient preferences, barriers, and wishes-including Advance Directives, Power of Attorney, Living Will, and DNR orders of various formats (e.g., EMS DNRs, POLST, POST, and MOLST).
