We are a privately hired Aging Life Care Practice serving central Florida. Our staff are members of the Aging Life Care Association with backgrounds in social work, nursing, or other health and social services related fields. Our mission is to provide expert guidance, advocacy, and care coordination through all phases of the aging process. We eliminate the indecision and trial and error of navigating the many choices in the aging care process. We provide continued monitoring, management, and advocacy to resolve problems as they arise. We work alongside caregivers, inside facilities, on behalf of a power of attorney or guardian.
Services:
ASSESSMENT & CARE PLAN – In-home review of client and environment and written report with recommendations
CARE MANAGEMENT – Screen, schedule, monitor third-party providers, set up and monitor medication management system in the home, purchase supplies as needed
PATIENT ADVOCACY – Escort clients to physician appointments to convey updates and changes, document physician orders, and establish follow-up
Advocate for client at the hospital, urgent care, and/or skilled nursing/rehab
AGING IN PLACE – Provide home safety assessment and recommendations on home alterations to improve livability and safety, coordinate services necessary to remain at home, including in-home care, home modifications and repair, security, lawn maintenance, utilities, groceries, etc.
FACILITY PLACEMENT – Determine appropriate facility options, tour with client, facilitate move-in
MOVE MANAGEMENT – Coordinate movers, utility set up, furniture and décor selection, packing, and setting up new room
DAILY MONEY MANAGEMENT – Monitor daily cash flow, reconcile accounts, assist with bill paying, gather documents for taxes, file claims, negotiate with creditors
DEMENTIA CARE CONSULTING – Provide strategies to manage behaviors outside of medication
