Our mission is to reduce unnecessary hospital readmissions by: improving patient prognosis post-hospitalization, adherence to discharge care plans, and slowing the rate of advancement of chronic disease. We do so by taking an active role in discharge planning, start of care, and exclusively working with caregivers who share our core values of sincerity, empathy, efficiency and teamwork.
