After defining the plan, the Roadmap for Aging team will connect individuals with referral partners who can assist with both immediate and future needs. This process empowers individuals to chart their own path forward, alleviating the burden on loved ones and caregivers who may be unsure of how to meet their needs, and establishing a robust support system.
