All About You AFC is a Massachusetts-based, nurse-led organization dedicated to helping families care for loved ones at home through the state’s Adult Foster Care (AFC) program. The agency provides Medicaid-funded support to caregivers, enabling individuals with physical or cognitive challenges to remain safely and comfortably in their own homes.
Founded by two registered nurses with extensive experience in community health and case management, All About You AFC offers a high-touch, personalized approach to care.
Core Services We Provide
Adult Foster Care Program: Provides financial support to caregivers, often family members or close friends, who assist individuals with daily living activities such as dressing, bathing, and mobility. Caregivers may receive a tax-free stipend of up to $1,500 per month per member.
Clinical Oversight: Each caregiver-member pair is supported by a dedicated registered nurse and case manager. The nurse assists with chronic disease management and navigating the healthcare system, while the case manager ensures that social determinants of health, such as housing, food, and transportation, are addressed.
Transportation Services: Operated by healthcare professionals, All About You Transportation offers reliable, compassionate rides for medical appointments, errands, and more. All staff are CPR and First Aid certified, ensuring safety and comfort during transit.
Insurance Assistance: The agency assists with MassHealth applications, renewals, and transitions to Senior Care Options plans, including Fallon, Tufts, United, and SWH.
Medical Equipment Support: Provides access to durable medical equipment and supplies, such as incontinence products, mobility aids, grab bars, and medication reminders, to support members’ daily needs.
