The Graham Team provides families with care solutions – realistic solutions specific to their unique needs. Costs are reviewed, as well as potential resources including Medicare, Medicaid, VA Benefits, and Long Term Care Insurance. Clinical needs and level of care are assessed, along with personal preferences, resulting in specific local care options to review and tour – options that make sense!
Jerry and Michelle each have over 20 years of experience in long term care. Their Team includes Certified Senior Advisors, Certified Information Referral Specialist in Aging, Nurses (LPN and RN), Qualified Assessors, and a Collaborative Law Allied Professional – Eldercare Specialist.
