At Vitality Hospice and Palliative Care we provide symptom management to those with advanced illness. We focus on the total unit of care, supporting both patients and their caregivers through the duration of care.
Hospice services are provided primarily at the patient’s residence by an interdisciplinary group (IDG) of professionals and volunteers. Hospice services are available both on a scheduled, and as needed basis, twenty-four hours a day, seven days a week. Medical supplies, equipment, and medications prescribed for the relief of pain and symptoms related to the terminal illness and related conditions are provided as determined by the interdisciplinary group.
