Adirondack Health Institute (AHI) is a non-profit organization founded in 1987, dedicated to improving health and healthcare services in the Adirondack region of New York. AHI collaborates with hospitals, physician practices, and community organizations to enhance equity, quality, access, and affordability of healthcare. Its mission is to ensure that every individual in the region can live their healthiest life.
AHI operates as a regional health improvement organization, focusing on population health, care coordination, and payment reform. It offers a variety of services, including a Health Home/Care Management Program that connects high-risk Medicaid members with essential medical and social supports. AHI also provides enrollment assistance for health insurance options and has implemented telehealth services to improve access to care in rural areas. By working with a network of healthcare providers, AHI supports a wide range of clinical services, including primary care, specialty care, rehabilitation, and behavioral health, all aimed at enhancing healthcare delivery for vulnerable populations.
